What Is Wegovy?

Wegovy is a brand-name prescription medication manufactured by Novo Nordisk. Its active ingredient is semaglutide — the same molecule found in Ozempic, but at a higher dose (2.4 mg weekly) and FDA-approved specifically for chronic weight management rather than type 2 diabetes.

Wegovy is a GLP-1 receptor agonist. It mimics the action of glucagon-like peptide-1, a hormone naturally produced in the gut after eating. GLP-1 receptor agonists act on receptors in the brain and digestive system to reduce appetite, increase feelings of fullness, and slow gastric emptying — all of which can contribute to reduced caloric intake over time.

Wegovy received FDA approval for chronic weight management in adults in June 2021, and later for adolescents 12 and older with obesity. It was the first GLP-1 medication approved specifically for weight management since liraglutide (Saxenda) in 2014, and represented a significant step change in clinical efficacy data for pharmacological obesity treatment.

FDA Approval and Who Qualifies

Wegovy is FDA-approved as an adjunct to reduced-calorie diet and increased physical activity for chronic weight management in adults who meet one of the following criteria:

• Body mass index (BMI) of 30 kg/m² or greater (obesity), or
• BMI of 27 kg/m² or greater (overweight) with at least one weight-related health condition — such as type 2 diabetes, high blood pressure, or high cholesterol

Eligibility for a Wegovy prescription is determined by a licensed clinician based on a complete medical evaluation. BMI is one consideration, not the sole criterion. Your clinician will also consider your medical history, current medications, contraindications, and individual clinical picture. How Wegovy Works

Wegovy activates GLP-1 receptors found in the hypothalamus (the brain region involved in appetite regulation), the gastrointestinal tract, and the pancreas. The primary mechanisms relevant to weight management include:

• Appetite suppression: GLP-1 receptor activation in the brain reduces hunger signals and increases satiety, making it easier to eat less without constant hunger.
• Slowed gastric emptying: Food moves more slowly from the stomach to the small intestine, extending feelings of fullness after meals.
• Reduced food reward signaling: Some evidence suggests GLP-1 receptor agonists may reduce the reward response associated with high-calorie foods.

Wegovy is intended for use alongside a reduced-calorie diet and increased physical activity — it is not a standalone solution and works best as part of a comprehensive lifestyle approach overseen by a licensed clinician.

Wegovy Dosing Schedule

Wegovy is administered as a once-weekly subcutaneous injection. The dose is gradually increased over approximately 16 weeks to reach the target maintenance dose of 2.4 mg weekly. The escalation schedule is designed to minimize gastrointestinal side effects during the adjustment period.

Typical dose escalation (as per prescribing information):

• Weeks 1–4: 0.25 mg once weekly
• Weeks 5–8: 0.5 mg once weekly
• Weeks 9–12: 1.0 mg once weekly
• Weeks 13–16: 1.7 mg once weekly
• Week 17 onward: 2.4 mg once weekly (maintenance dose)

If side effects are problematic during escalation, a clinician may extend the time at a lower dose before advancing. Dosing decisions are made by the prescribing clinician based on your clinical response and tolerability.

Clinical Data: STEP Trials

Wegovy's FDA approval is supported by data from the STEP (Semaglutide Treatment Effect in People with Obesity) clinical trial program, a series of large randomized controlled trials conducted in tens of thousands of adults.

• STEP 1: Adults without diabetes — average body weight reduction of approximately 14.9% over 68 weeks with semaglutide 2.4 mg vs. 2.4% with placebo.
• STEP 2: Adults with type 2 diabetes — average body weight reduction of approximately 9.6% vs. 3.4% with placebo.
• STEP 3: Combined with intensive behavioral intervention — average weight reduction of approximately 16% vs. 5.7% with placebo.
• STEP 4: Weight regain following discontinuation was studied, showing that stopping treatment was associated with significant weight regain over time.

Wegovy Cost and Insurance Coverage

Wegovy's list price is approximately $1,300–$1,450 per month, varying by dose. This is the price before insurance, discounts, or savings programs. What you actually pay depends heavily on your insurance coverage and eligibility for manufacturer savings programs.

Insurance coverage for Wegovy varies. Some commercial insurance plans cover it; many exclude weight-management medications. Medicare Part D typically does not cover drugs approved only for weight loss, though this is changing with new legislation. Prior authorization is often required where coverage exists.

Novo Nordisk offers a savings program for Wegovy for eligible commercially insured patients — but this is not available to people with government insurance (Medicare, Medicaid). Terms change over time; verify directly with Novo Nordisk or your pharmacy.
